Please note: if you have not attended a Wellsprings Orientation, please click here to sign up to attend.  Attending an orientation is required before receiving a freedom session.  (Feel free to continue to request a session below...just be sure to attend an orientation sometime before your scheduled session.  We will follow up to let you know when the next one is scheduled.)

If you know someone who serves on a Wellsprings team and would prefer that they NOT be involved in your session for any reason, please indiciate their name(s) below. 
Sometimes close friends or family may not want to participate in each other's sessions for any number of reasons.  If it does not matter to you which team members are present, or if you do not know any team members by name, just leave the field blank.
 
Additional Comments
i.e, more about you, why you are requesting a session, etc.  This is optional and will only be seen by the Wellsprings leadership.
